The Patient Support Associate will connect with patients on a 1-1 basis. They will use an Electronic Health Record system and support the clinic’s rollout of the new EHR methods. The Patient Support provides administrative support.
The Patient Support Associate will focus on scheduling and screening patients.
The Patient Support Associate will focus on health maintenance of the health patients, patient support, and contact patients via phone call.
They will be mentored by Central Valley Health Network staff, and by the Community Clinic’s staff. There will be peer mentorship via committee tasks and reflection activities.
Successful completion of the program is 1700 hours and at least 42 weeks. Position starts as soon as the candidate and the clinic is available.
Daily responsibilities include linking infrequent clinical users who are uninsured to health coverage that they may be eligible for. They will support the effort of Population Health Outreach. They will handle patient complaints by following protocol while attempting to resolve the complaint. They will update monthly health plan membership roster data with Practice Management System.
The typical work environment contains desk work in an office, there may be some outreach events available.
The required qualifications for the ideal candidate would be to be 18+, High School/GED, US citizen/Lawful Permanent Resident.
